Chronic disease management wellness clinic
operation in a workplace clinic
part time or full time hours Monday through Thursday
Strong wellness and disease management focus
Experience with Diabetes and hypertension
Some acute/episodic care with focus on triage to primary care physicians
Utilization of Healthgram(tm) - proprietary Electronic Medical Record, to log chart reviews
Phone and in-person collaboration with an Mass. licensed Supervising Physician
Phone and e-mail collaboration with Medical Director (very minimal)
Wellness education experience is a strong plus!


Requirements:

MSN- masters of science in nursing
graduation from an accredited NP program as either ANP (adult nurse practitioner) or FNP (family nurse practitioner)
Active licensure in the state in which our clinic is located
National certification from either the AANP (American Academy of Nurse Practitioners) or ANCC (American Nurses Credentialing Center - which is the certifying branch of the ANA [American Nurses Association])

Nurse Practitioners need to have their own malpractice.
In regards to skill set the ideal candidate will have experience with Diabetes, Hyper tension, Hyperlipidemia and experience with one on one lifestyle counseling.
